Mole Valley Life are delighted to have had their Hospital to Home Support Service (HTHSS) extended until 31st March 2023 to help support older and vulnerable residents as they are discharged from hospital in the Surrey Downs ICP area. Mole Valley Life’s HTHSS will now also be taking self-referrals, making the service easily accessible to all Mole Valley residents who may benefit from it.
Since 2018, via the HTHSS, the Mole Valley Life team have helped to support and advise over 800 residents with practical tasks when they return home from a hospital stay. The HTHSS can provide:
· Up to 6 weeks of support
· Use of a Lifeline alarm for up to 6 weeks
· Regular telephone welfare calls to monitor your health and wellbeing
· Signposting to information and assistance services
· Referral to befriending services and social activities*
· Emergency food pack or shopping service upon discharge (if required)*
· Referral to meal services and prescription collections*

Mole Valley Life is operated by Mole Valley District Council and provides the Community Transport, Social Centre and Lifeline Alarm and TEC services for the Mole Valley area. Alongside this, Mole Valley Life work in partnership with with Reigate & Banstead Borough Council, Spelthorne Borough Council and Tandridge District Council to deliver the Lifeline Alarm and TEC service for Reigate & Banstead, Spelthorne and Tandridge residents.
